Declaration for Pension

State of New York, County of Fulton, ss;

On this 5th day of May, A. D. one thousand nine hundred and seven personally appeared before me, a Clerk of the Surrogate's Court within and for the County and State aforesaid, Frank Isen, who being duly sworn according to law, declares that he is 73 years of age, and a resident of Johnstown, County of Fulton, State of New York; and that he is the identical person who ENROLLED at Springfield, N.Y., under the name of Frank Isen on the 18th day of January, 1864, as a private, in Co. B. 24th Regiment Cavalry NY Vols. and was afterwards transferred to Co. B. 1st NY Provisional Cavalry, in the service of the United States, in the Civil War, and was HONORABLY DISCHARGED at Syracuse, N. Y., subsequent to the 19th day of July, 1865 when he was mustered out from service of last mentioned company at Cloud Mills, Va. as Frank Isan.

That he was not employed in the military or naval service of the United States otherwise than as stated above.  That his personal description at enlistment was as follows, to be of recollection:  Height 6 ft, --- inches; complexion dark; color of eyes dark; color of hair dark; that his occupation was a farmer; that he was born September 12, 1833 at Hanover, Germany.

That his several places of residence since leaving the service have been as follows:
came to Johnstown in 1866, and he lived there or in the immediate vicinity ever since.

That he receives a pension under pension certificate No. 1016911.

That he makes this Declaration for the purpose of being placed on the Pension Roll of the United States, under the provisions of the Act of February 6, 1907.

That he appoints, with full power of substitution and revocation, J. Keck of Johnstown, County of Fulton, State of New York, his true and lawful attorney, to prosecute his claim, and requests and directs that he be allowed and paid, upon the issuance of a Certificate, or thereafter, such fee as may be hereafter provided by law, NOT EXCEEDING TEN DOLLARS.

Declaration for Widow's Pension
Act of April 19, 1908

State of New York, County of Fulton, ss;

On this 5th day of May, A. D. one thousand nine hundred and nine personally appeared before me, a Clerk of the Surrogate's Court within and for the County and State aforesaid, Elizabeth J. Isen, aged sixty-five years, a resident of Johnstown, County of Fulton, State of New York; who, being duly sworn according to law, makes the following declaration in order to obtain under the provisions of the ACT OF CONGRESS APPROVED APRIL 19, 1908.

That she is the widow of Frank Isen at Utica, N. Y. as I am informed, as a Private, in Co. B. 24th Regiment Cavalry NY Vols. and after in Co. B. 1st NY Provisional Cavalry, and HONORABLY DISCHARGED July, 1865 having served ninety days or more during the late war of the rebellion.  That the soldier was not in the military or naval service of the United States except as stated above.  Am unable to give date of enlistment, or exact date of discharge.  I understand that said soldier enlisted at Utica, N. Y. as above set forth, but his discharge has been lost.

That she was married under the name of Elizabeth J. Reese to the said soldier at Fort Plain, N. Y., on the 3rd day of September, 1865 by Rev. H. H. Baker; that there was no legal barrier to the marriage; that she had not been previously married; that the soldier had not been previously married.

That the said soldier died April 30, 1909 at Johnstown , N. Y., that she was not divorced from him; that she has not remarried since his death.

That the names and dates of birth of all children of the soldier, now living and under sixteen years of age are as follows:
No children under sixteen years of age.  Four children born of said marriage.

That she has not herefore applied for pension, Said soldier was enrolled under pension Certificate No. 1016911, Buffalo Agency.

That she hereby appoints, with full power of substitution and revocation, J. Keck of Johnstown, her true and lawful attorney, to prosecute his claim, and directs the sum of Ten Dollars be paid him for his services.

That her post address is No. 34 Fisher Avenue, Johnstown, County of Fulton, State of New York.
