Declaration for Pension

State of New York, County of Jefferson, ss:
On this 29th day of April, A. D. one thousand nine hundred and thirteen, personally appeared before me, a Notary Public within and for the county and State aforesaid, Oliver Perry, who, being duly sworn accordingly to law, declares that he is 70 years of age, and a resident of Alexandria Bay, county of Jefferson, State of New York; and that he is the identical person who was enrolled at Johnstown, New York, under the name of Oliver H. Perry, on the 28th day of August, 1862, as a private in Co. D, 153 rd. Reg. N Y State Volunteers, as a private in the service of the United States, in the Civil war, and was HONORABLY DISCHARGED at New York City, N. Y., on the 7th day of October, 1864.
That he also served in 10th Co., I Battalion Veteran Reserve Corps.
That he was not employed in the military or naval service of the United States otherwise than as stated above.. That his personal description at enlistment was as follows:
Height, 5 feet 9 inches;
Complexion, Fair;
Color of eyes, Grey;
Color of hair, Sandy;
That his occupation was Laborer,
That he was born April 28th, 1843, at Johnstown, N. Y.
That his several places of residence since the service have been as follows:
Johnstown to Brier, R111, N. Y. 1877 to Alexandria Bay, N Y 1881
That he is a pensioner under under certificate No. 51488. That he has _________ applied for pension under original No. ________.
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912.
That his post office address is Alexandria Bay, county of Jefferson, State of New York.
